$379/year for initial plan member and $279 for each additional member. Each member on plan must live in the same household.
BENEFITS
We offer two convenient ways to break up your payments, making dental care easy on your budget. You can choose to pay for half of your treatment at the time of service and the remainder the following month. You can also opt to make four monthly payments with a 5% interest charge. With this option, the charge is made automatically the same day each month for four consecutive months. With both options, you must have a card on file with us.
